8D problem solving: a practical guide

The 8D method is the common language of supplier corrective action. Used well, it removes causes. Used as a form, it produces paperwork.

01What 8D is

8D, eight disciplines, is a structured problem solving method widely used in automotive and industrial supply chains to respond to customer complaints and significant internal problems. Its strength is the combination of immediate containment with a disciplined search for root causes and verified permanent actions.

02The eight disciplines

  1. D1 Team. A small cross-functional team with process knowledge and authority. For supplier problems include production, quality and engineering, not only the quality manager.
  2. D2 Problem description. Facts only: what, where, when, how many, how detected. Use is / is not analysis to bound the problem.
  3. D3 Containment. Protect the customer now: stock at the supplier, in transit and at the customer is quarantined, sorted or reworked. Record quantities.
  4. D4 Root cause. Find and verify the root cause of occurrence and the root cause of non-detection (escape).
  5. D5 Permanent corrective actions. Choose actions that remove the verified causes and confirm they will work.
  6. D6 Implementation and validation. Implement, remove containment when data shows effectiveness.
  7. D7 Prevention. Update FMEA, control plan, work instructions and similar products and processes.
  8. D8 Closure. Recognise the team and close the report.

03Two root causes, not one

Most weak 8D reports analyse only why the defect was made. Equally important is why it was not detected before shipment. A missing gauge, an inadequate sampling plan or an unclear acceptance criterion is a root cause of escape and needs its own action.

Ask both questions with the 5 Why method, separately.

04Signs of a weak 8D

  • Root cause given as 'operator error' or 'human mistake'.
  • Corrective action is 'operator retrained' or 'warned'.
  • No data showing that the cause was verified, for example by reproducing the defect.
  • Containment quantities missing.
  • FMEA and control plan not updated.

05What buyers should do

Review 8Ds critically and verify on site. Effective corrective action is visible on the shop floor: a poka-yoke device, a changed fixture, a new gauge, a revised parameter window. QUALITAS TR provides this verification through NCR and CAPA management.
